How Can a Post-Hospital Rehabilitation Center Help You Recover?

Post-hospital rehabilitation centers work with you, your family, and your existing medical team to supervise your care.  Doctors, registered nurses, nursing aides, dieticians, physical, occupational and speech therapists, are only a few of the qualified professionals at post-hospital rehabilitation centers that help aid your recovery. Post-hospital rehabilitation centers work with you to understand your needs, goals and expectations and help make you the strong independent person you aim to become once again.

What will happen during your stay?
  • The post-acute rehabilitation center’s doctor will work with your existing medical team and the medical team at the center to oversee your care.
  • Registered nurses and nursing aides will care for you around the clock, according to your doctor’s orders, tending to your medication needs, monitoring for problems, and checking to ensure you are making progress and on your way to meeting your health care goals.
  • Physical therapists will teach you how to make your muscles stronger and employ exercises and techniques to help you meet your goals. Physical therapy is focused on helping you learn to maneuver stairs safely, get in and out of vehicles, sit down safely on a chair or toilet, balance and use assistive devices such as crutches, a walker or cane appropriately.
  • Occupational therapists will help you relearn the tasks that you perform each day at home. You might relearn how to use a knife safely, cook, do laundry, etc.  Their goal is to ensure you are able to perform activities of daily living safely and independently, once you return home.
  • Speech and language therapists will evaluate problems with speech and swallowing.
  • A dietitian will work with you to ensure any special dietary needs are met.

The ultimate goal is to care for you while you're recovering and help you regain the strength to safely return to your prior living situation.
